NASWC MEMBERSHIP APPLICATION

Instructions:
1. Fill out the form online.
2. Print it.
3. Sign it and send it in with your dues.

Select either a Regular membership or an Over 65 membership, when the head-of-household is sixty-five (65) years of age or older. Only one membership per household is needed.

Regular - $25.00 Over 65 - $20.00

Membership fees are due on October 1st of each calendar year. Statements will be mailed in September.

For first time members only who join before October 1st your dues are prorated per quarter as follows:

100% October-December 75% January - March 50% April - June 25% July - September

For example, if you mail your application on May 15th and are over 65, you would pay 50% of the $20 fee ($10).
